Abstract:Objective To investigate the correlation of blood uric acid, blood lipid and 25-OH vitamin D with recurrence of benign paroxysmal positional vertigo(BPPV).Methods A total of 239 BPPV patients who were treated in Zhengzhou People's Hospital from January 2021 to June 2023 were selected as the study objects.According to the recurrence situation 6 months after clinical cure, the patients were divided into the recurrence group(73 cases)and the non-recurrence group(166 cases).Blood uric acid,25-OH vitamin D,and blood lipid of triglyceride(TG),total cholesterol(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),apolipoprotein A1(ApoA1)and lipoprotein a(Lpa)were detected in both groups. Results The levels of blood uric acid and lipid indexes(TG,TC,LDL-C and Lpa)in the recurrence group were higher than the non-recurrence group, while the levels of HDL-C,ApoA1 and 25-OH vitamin D were lower than the non-recurrence group(P<0.05).Logistic regression analysis showed that BPPV recurrence was closely related to serum uric acid,TG,HDL-C,ApoA1 and 25-OH vitamin D(P<0.05).The results of the logistic regression analysis showed that the recurrence of BPPV was associated with blood uric acid(OR=1.865,95%CI:1.002-3.471),triglycerides(OR=2.474,95%CI:1.101-5.559),HDL-C(OR=0.427,95%CI:0.220-0.829),ApoA1(OR=0.520,95%CI:0.273-0.991),and 25-OH vitamin D(OR=0.318,95%CI:0.147-0.687)(P<0.05).Receiver operating characteristic curve analysis was showed that blood uric acid,TG,HDL-C,ApoA1 and 25-OH vitamin D had certain clinical value in predicting BPPV recurrence(P<0.05),and the area under the curve was between 0.829 and 0.985.Conclusion Blood uric acid, blood lipids(TG,HDL-C,ApoA1)and 25-OH vitamin D were important indicators affecting BPPV recurrence,so clinical monitoring of the above indicators should be emphasized to assess the risk of BPPV recurrence. At the same time, intervention and management measures should be developed to actively control blood uric acid, blood lipids and 25-OH vitamin D within the normal range to prevent disease recurrence.
