Application of Evoked Potential Combined with Coma Recovery Scale-Revised in the Prognosis Evaluation of Children with Prolonged Disorders of Consciousness

Abstract:Objective To investigate the clinical value of somatosensory evoked potential(SEP)and auditory evoked potential(BAEP)combined with coma recovery scale-revised(CRS-R)in the prognosis assessment of children with prolonged disorders of consciousness(pDOC).Methods The clinical data of 51 children with pDOC were retrospectively analyzed,and the endpoint was coming out of minimally conscious state,death or admission for 6 months.Whether consciousness was restored or not was used as the outcome index,and the differences in age,gender,etiology,disease course,CRS-R score,SEP results,and BAEP results in the consciousness group and those in the non-consciousness group were compared.With the recovery of consciousness as the dependent variable,multivariate logistic regression was performed on the preliminarily screened variables to establish a regression model,and receiver operating characteristic(ROC)curve was drawn to evaluate the predictive efficiency.Results There were significant differences in age,course,SEP grade,BAEP grade and CRS-R score between the two groups(P<0.05).Multivariate logistic regression found that the independent variable course of disease,CRS-R score,and SEP grade were correlated with the outcome of children at 6 months of admission(P<0.05).The shorter the course of disease,the lower the SEP grade and the higher the CRS-R score,the more likely the consciousness was to recover.The ROC curve was drawn,and the combined area under the curve(AUC)value of the three indexes was 0.950(P<0.05),indicating that the combined evaluation of the three indexes had the highest accuracy in the recovery of consciousness in children with pDOC.Conclusion SEP grade,CRS-R score and disease course were independent factors influencing the prognosis of pDOC children,and the combined sensitivity and specificity of the three indexes were preferable,which had high predictive value.
