Prognostic Risk Factors and Predictive Model Construction of Oral Implant Restoration in Patients with Dentition Defect

Abstract:Objective To explore the prognostic risk factors of oral implant restoration in patients with dentition defect,and to construct a predictive model.Methods Retrospective collection of data from 94 patients with dentition defect who underwent oral implant restoration at Xinyang Central Hospital from January 2020 to January 2024.Based on the observation of implant survival three months after oral implant restoration surgery,patients who did not survive were included in the poor prognosis group,while those who survived were included in the good prognosis group.The clinical data of two groups of patients were compared,multiple logistic regression analysis was used to identify the risk factors affecting the prognosis of dental implant restoration in patients with dentition defect,and based on the obtained risk factors,R software and rms package were used to construct a column chart prediction model and decision curve.Results Among the 94 patients with dentition defect who received oral implant restoration,18 implants did not survive and were included in the poor prognosis group,with an incidence of 19.15%.The results of univariate analysis showed that smoking history,periodontitis,number of missing teeth ≥3,diabetes mellitus and residual binder may be risk factors for the prognosis of oral implant restoration in patients with dentition defect(P<0.05).The risk factors obtained from univariate analysis were included in multivariate logistic regression analysis.The results showed that smoking history,periodontitis,missing teeth≥3,and diabetes were risk factors for the prognosis of oral implant restoration in patients with dentition defect(OR>1,P<0.05).Based on the risk factors obtained by logistic regression analysis and their corresponding OR values,a nomogram prediction model was constructed.It was found that patients with a history of smoking,periodontitis,missing teeth≥3,and diabetes had a higher risk of poor prognosis of oral implant restoration.Standard curve was drawn,suggesting that the nomogram prediction curve was similar to the Y-X straight line;the receiver operating characteristic(ROC)curve was drawn,and the area under the curve(AUC)of the nomogram model for predicting the poor prognosis of oral implant restoration in patients with dentition defect was 0.858.The decision curve showed that the maximum net benefit was 0.191.The model had good accuracy,discrimination and good clinical practicability.Conclusion Smoking history,periodontitis,number of missing teeth≥3 and diabetes mellitus were risk factors for poor prognosis of patients with dentition defect after oral implant restoration.The nomogram prediction model based on the above factors had good consistency and discrimination,which could help to evaluate the risk of poor prognosis of patients.
