Influencing Factors and Predictive Model Construction of Postoperative Urinary Control Function Recovery in Patients with Benign Prostatic Hyperplasia

Abstract:Objective To explore the influencing factors of postoperative urinary control function recovery in patients with benign prostatic hyperplasia,and to construct a predictive model for poor postoperative urinary control function recovery.Methods A retrospective analysis was conducted to collect data from 198 patients with benign prostatic hyperplasia diagnosed and treated with surgery at the First Affiliated Hospital of Henan University of Science and Technology from March 2021 to December 2023.The recovery of urinary continence function after transurethral resection of the prostate(3 months after review)was recorded.According to the recovery of urinary continence function after operation,the patients were divided into good recovery group and poor recovery group.The baseline data of the two groups were compared,and the influencing factors of postoperative urinary continence function recovery in patients with benign prostatic hyperplasia were analyzed.A nomogram model was constructed based on the influencing factors.Results Among 198 patients with benign prostatic hyperplasia who underwent transurethral resection of the prostate,84 had poor recovery of urinary control function,accounting for 42.42%.A preliminary comparison of the baseline data between the poor recovery group and the good recovery group showed that the poor recovery of urinary control function in patients with benign prostatic hyperplasia after surgery might be related to the patients with hypertension,diabetes,preoperative detrusor function,preoperative residual urine volume,and postoperative indwelling catheter time(P<0.05).After a single logistic regression analysis,the multivariate logistic regression model was co constructed.It was found that hypertension,diabetes,preoperative detrusor dysfunction,more residual urine volume before surgery,and a longer indwelling catheter after surgery might be risk factors for poor recovery of postoperative urinary control function in patients with benign prostatic hyperplasia(OR>1,P<0.05).Based on the risk factors,a nomogram prediction model was constructed,and the Bootstrap method was used to verify the model internally.The fitting between the internal calibration curve and the ideal curve was good.The receiver operating characteristic(ROC)curve showed that the overall model predicts an area under the curve(AUC)of 0.952,indicating good accuracy in model evaluation.Conclusion Preoperative hypertension,diabetes,decreased detrusor function,more residual urine volume and longer indwelling catheter after operation may be risk factors for poor recovery of urinary control function in patients with benign prostatic hyperplasia.
