Application of Patient-Reported Outcomes in Evaluating the Efficacy of Percutaneous Internal Fixation for Thoracolumbar Burst Fractures

Abstract:Objective To use patient-reported outcome measures(PROM)to evaluate the therapeutic efficacy of percutaneous internal fixation in patients with thoracolumbar burst fractures and to compare the correlations between different PROMs.Methods A retrospective analysis was conducted on 61 patients who underwent percutaneous internal fixation for thoracolumbar fractures(surgery group)and 47 patients managed with conservative treatment(control group).General clinical data and postoperative complications were collected.All patients were assessed using PROMs at 6 months postoperatively or after conservative treatment.Three PROMs were used:the numeric rating scale(NRS),the Oswestry disability index(ODI),and the EuroQol five-dimensional questionnaire(EQ-5D-5L).Results The surgery group showed significantly lower NRS scores and ODI scores compared to the control group(P<0.05).The visual analog scale(VAS)score and the EQ-5D-5L health utility index were significantly higher in the surgery group than in the control group(P<0.05).Among the surgery group patients,correlations were found between the three PROMs:the EQ-5D-5L health utility index correlated negatively with the NRS(r=-0.732,P=0.014)and ODI(r=-0.815,P=0.003);the EQ-5D-5L pain/discomfort dimension correlated positively with the NRS(r=0.823,P=0.004)and ODI(r=0.844,P=0.002);the NRS correlated positively with the ODI(r=0.803,P=0.005);and the EQ-5D-5L anxiety/depression dimension correlated positively with the ODI(r=0.743,P=0.005)and the NRS(r=0.749,P=0.042).Conclusion PROMs can effectively evaluate the outcomes of both conservative treatment and percutaneous internal fixation for stable thoracolumbar burst fractures,with percutaneous internal fixation demonstrating superior clinical efficacy.The NRS,ODI,and EQ-5D-5L are correlated with each other,and their combined use in clinical practice can effectively assess clinical treatment outcomes.
