Impact of Surgical Related Factors on Restenosis After Stent Placement for Long Segment Superficial Femoral Artery Lesions

Abstract:Objective To analyze the impact of surgical related factors on in-stent restenosis(ISR)after stent placement for long segment superficial femoral artery(SFA)lesions.Methods Basic information and surgical data of 99 patients with long segment[Trans-Atlantic Inter-Society Consenses(TASC Ⅱ)C/D]SFA lesions treated in the Department of Vascular Surgery of the First Affiliated Hospital of Zhengzhou University from January 2020 to November 2022 were collected.After one year of follow-up,the patients were divided into the restenosis group(28 cases)and the non-restenosis group(71 cases)based on whether ISR occurred.The basic information and surgical data of the two groups were compared.Results There were statistically significant differences in age,infrapopliteal artery≤1,tibiofibular trunk artery lesion,and overlapping length of stents between the two groups(P<0.05).There were no statistically significant differences in gender,hypertension,diabetes,coronary heart disease,cerebral infarction,smoking history,alcohol consumption history,disease duration,popliteal artery lesions,use of braided stent,local extravasation,retrograde puncture,stent length,stent coverage length,and number of stents between the two groups(P>0.05).Multivariate logistic regression results suggested that infrapopliteal artery ≤ 1,tibiofibular trunk lesion,and stent overlap length were independent risk factors for ISR after SFA long segment lesions stent surgery.Conclusion When treating long segment SFA lesions with stent placement,increasing the outflow pathway below the knee and reducing stent overlap may have a positive impact on reducing the occurrence of ISR.
