Distribution and Drug Resistance of Nosocomial Infection Pathogens in Critical Departments of Zhengzhou People's Hospital from 2019 to 2022

Abstract:Objective To analyze the distribution and drug resistance of nosocomial infection pathogens in critical departments of Zhengzhou People's Hospital from 2019 to 2022,so as to provide reference basis for clinical rational use of antibiotics and prevention and control of nosocomial infection.Methods The clinical data of 311 patients with nosocomial infection in the critical department of the hospital from 2019 to 2022 were reviewed retrospectively,the infection site,pathogen species and drug resistance were analyzed.Results Among 7 930 hospitalized patients in intensive care departments,311 patients had infection and 357 times of infection.Among multiple intensive care unit(ICU)specialties,the hospital infection rate in the neurology intensive care unit is the highest,at 7.94%.The most common infection site of inpatients in critical departments was the lower respiratory tract,accounting for 41.74%,followed by the urinary system,accounting for 24.09%.A total of 357 strains of pathogens were detected,the proportion of Gram negative bacteria was the highest,at 73.67%,and the top three pathogens were Klebsiella pneumoniae(21.07%),Escherichia coli(18.54%)and Acinetobacter baumannii(14.57%).The resistance rates of Acinetobacter baumannii and Klebsiella pneumoniae to carbapenems were 42.31%and 38.67%,respectively.They were more sensitive to polymyxin B and cefoperazone/sulbactam,while Escherichia coli was more sensitive to cefoperazone/sulbactam and piperacillin/tazobactam.The detection rate of methicillin-resistant Staphylococcus aureus accounts for 29.17%,most Staphylococcus aureus was sensitive to moxifloxacin.Conclusion The hospital infections of patients in the intensive care department of Zhengzhou People's Hospital are mainly caused by Gram negative bacteria,while multidrug-resistant bacteria are mainly carbapenem resistant Acinetobacter baumannii and carbapenem resistant Klebsiella pneumoniae.Clinically,personalized anti infective treatment with antibiotics should be standardized based on pathogen resistance,and various departments in hospitals should strengthen the monitoring of nosocomial infections and the detection of drug-resistant bacteria.
