Application value of EB Virus DNA and Immune Indicators in the Evaluation of the Condition of Children with Infectious Mononucleosis

Abstract:Objective To explore the application of Epstein-Barr virus(EBV)DNA and immune indicators in children with infectious mononucleosis(IM)and their evaluation value for the condition of the children.Methods A total of 170 children with IM who were hospitalized in the First Affiliated Hospital of Zhengzhou University from December 2019 to May 2024 were selected as the research subjects,and were divided into a mild group(107 cases)and a severe group(63 cases)according of their illness.Baseline data,EBV antibodies,EBV-DNA,and immune indicators[immunoglobulin(IgG,IgA,IgM),complement(C3,C4),T lymphocyte subsets(CD3+,CD4+,CD8+,CD4+/CD8+)]were compared between the two groups of children.The data were analyzed by SPSS 27.0,and the receiver operating characteristic(ROC)curve was used to analyze the evaluation value of individual and combination of multiple indexes for the condition of IM children.Results The proportion of children with eyelid edema,liver enlargement,and spleen enlargementin the severe group was higher than that in the mild group,and the differences were statistically significant(P<0.05).The levels of EBV-DNA,IgG,IgA,IgM,CD3+,and CD8+in the severe group were higher than those in the mild group,while the levels of CD4+and CD4+/CD8+were lower than those in the mild group,with statistically significant differences(P<0.05).The ROC curve analysis showed that the area under the curve(AUC)values of EBV-DNA+immunoglobulin+T lymphocyte subsets,EBV-DNA,and EBV-DNA+T lymphocyte subsets were higher,with AUC values of 0.741,0.732,and 0.721,respectively,with sensitivities of 71.9%,71.4%,and 65.0%,respectively,the specificity was 76.6%,59.8%,and 71.8%respectively.The sensitivity(71.9%)and specificity(76.6%)of EBV-DNA combined with immunoglobulin and T lymphocyte subsets in evaluating the severity of IM in children were superior to those of single indicators.Conclusion EBV-DNA and immune indexes are related to the severity of IM children's illness,and the combination of multiple indexes is helpful to monitor the disease progress of IM children,so as to accurately evaluate illness and make timely diagnosis and treatment.
