Current Status and Influencing Factors of Burden Level of Primary Caregivers of Dual Targeted Therapy for HER2 Positive Breast Cancer

Abstract:Objective To investigate the burden level of primary caregivers of dual targeted therapy for HER2 positive breast cancer and analyze its influencing factors.Methods A total of 180 primary caregivers of patients with HER2-positive breast cancer who received double-targeted therapy in the First Affiliated Hospital of Henan University of Traditional Chinese Medicine from July 2020 to July 2023 were retrospectively selected to be included in the study.The current status of primary caregiver burden was investigated by Zarit caregiver burden scale(ZBI).During the same time,clinical data of breast cancer patients and their main caregivers were collected,and their ZBI scores were compared.Multivariate logistic regression method was used to analyze the influencing factors of the burden level of main caregivers.Results The total ZBI score of 168 main caregivers of breast cancer patients was(34.57±8.39),among which the score of responsibility burden was(11.56±2.64),and the score of personal burden was(22.43±5.36).Severity of burden:72 cases were none/mild,accounting for 42.85%;60(35.71%)were moderate;36 cases were severe,accounting for 21.43%.The proportion of women in the high burden primary caregivers,the proportion of employment in the working state,the proportion of self-paid medical expenses,the proportion of family monthly income less than 10 000 yuan,and the proportion of continuous care time more than 3 months were higher than those of the low burden primary caregivers,the proportion of assisting caregivers,and the proportion of Barthel index(BI)of breast cancer patients more than 60 were lower than those of the low burden primary caregivers.The difference was statistically significant(P<0.05).Binary logistic regression analysis showed that gender(OR=3.471),payment method of medical expenses(OR=4.275),family monthly income(OR=2.857),and continuous care time(OR=2.463)were independent influencing factors of the burden level of the main caregivers(P<0.05).Conclusion The main caregivers of HER2 positive breast cancer with dual targeted therapy have a high burden level.Female,self-paid medical expenses,family monthly income<10 000 yuan,and care duration>3 months are risk factors for the burden level of the main caregivers.Therefore,clinical workers should pay great attention to this group of people,and take effective measures to intervene in time to reduce the burden of the main caregivers and prevent the occurrence of burden.
