Curative Effect and Recurrence Influencing Factors of Irinotecan Neoadjuvant Chemotherapy Combined with Laparoscopy in the Treatment of Locally Advanced Cervical Cancer

Abstract:Objective To explore the efficacy of irinotecan neoadjuvant chemotherapy combined with laparoscopy in the treatment of locally advanced cervical cancer,and to analyze the influencing factors of recurrence.Methods A total of 86 patients with locally advanced cervical cancer diagnosed and treated in our hospital from January 2018 to October 2023 were divided into control group(43 cases,concurrent chemoradiotherapy)and study group(43 cases,concurrent chemoradiotherapy+irinotecan).The clinical efficacy,postoperative adverse reactions and risk factors of postoperative recurrence were compared between the two groups.Results Compared with 60.47%in the control group,81.40%in the study group was higher(P<0.05).Compared with the control group,the incidence of thrombocytopenia,neutropenia,abnormal liver function,alopecia,diarrhea,nausea and vomiting in the study group was lower(P<0.05).Compared with the control group(35.56%),the recurrence rate of the study group(6.98%)was lower(P<0.05).By univariate analysis,the tumor diameter,differentiation degree,infiltration depth,pelvic lymph node metastasis,D-dimer(D-D),squamous cell carcinoma antigen(SCC-Ag)and treatment methods in recurrent group were statistically significant compared with those in non-recurrent group(P<0.05).Logistic regression analysis showed that tumor diameter ≥4 cm,invasion depth ≥1/2,pelvic lymph node metastasis,D-D≥0.5 mg·L-1,and only concurrent radiotherapy and chemotherapy were the risk factors for postoperative recurrence(P<0.05),and high differentiation was the protective factor for postoperative recurrence(P<0.05).Conclusion Irinotecan neoadjuvant chemotherapy combined with laparoscopy is effective in the treatment of locally advanced cervical cancer,which can reduce postoperative adverse reaction rate and postoperative recurrence rate.Postoperative recurrence is related to tumor diameter,infiltration depth,pelvic lymph node metastasis,D-D and treatment methods.
