Predictive Value of Serum ALB and MCP-1 Levels for Respiratory Failure in Patients with Severe Pneumonia

Abstract:Objective To explore the predictive value of serum albumin(ALB)and monocyte chemokine-1(MCP-1)in severe pneumonia patients with respiratory failure.Methods A retrospective study was conducted to select the clinical data of 98 patients with severe pneumonia admitted to Nanyang Second General People's Hospital from May 2021 to February 2023.The clinical data of 98 patients with severe pneumonia were reviewed.The data of 24 patients with respiratory failure during hospitalization were included in the occurrence group,and the data of 74 patients without respiratory failure were included in the non occurrence group.The baseline data were recorded by reviewing the medical records of patients with severe pneumonia,and the serum MCP-1 and ALB levels of all patients on the day of admission were collected.The receiver operating characteristic(ROC)curve was used to explore the predictive value of serum MCP-1 and ALB for respiratory failure in patients with severe pneumonia.Results The levels of MCP-1 and arterial partial pressure of carbon dioxide(PaCO2)in the occurrence group were higher than those in the non occurrence group,while the levels of arterial partial pressure of oxygen(PaO2)and arterial oxygen saturation(SaO2)were lower than those in the non occurrence group,with a statistical significant difference(P<0.05).The results of point two column correlation showed that serum MCP-1 and ALB were associated with respiratory failure in severe pneumonia patients(P<0.05).The ROC curve was plotted,and the results showed that the area under the curve(AUC)of serum MCP-1,ALB,and their combination for predicting respiratory failure in severe pneumonia patients were 0.814,0.790 and 0.883,respectively,which had certain predictive value,and the combined predictive value was the highest.Conclusion Serum MCP-1 and ALB can better predict whether severe pneumonia patients will develop respiratory failure.The higher the serum MCP-1 value and the lower the ALB value,the greater the likelihood of patients developing respiratory failure.
