Risk Factors of Cervical Sagittal Imbalance After Posterior Cervical Expansive Open-Door Laminoplasty

Abstract:Objective To investigate the risk factors of cervical sagittal imbalance(CSI)after posterior cervical expansive open-door laminoplasty(EODL).Methods A retrospective study was conducted on 81 patients who underwent posterior C3-6 segmental expansive open-door laminoplasty for multilevel cervical spondylotic myelopathy(MCSM)from January 2018 to January 2023 at the First Affiliated Hospital of Zhengzhou University.There were 56 males and 25 females,with a mean age of(56.59±10.82)years.The patients were divided into an imbalance group(20 cases)and a balanced group(61 cases)according to the presence or absence of sagittal imbalance at the final postoperative follow-up,and the age,gender,body mass index,preoperative C2-C7 Cobb angle,C0-C2 angle,T1-slope,C2-C7 sagittal vertical axis(C2-C7SVA),center of gravity of the head-C7 sagittal vertical axis(CGH-C7SVA),neck tilt(NT),thoracic incidence angle(TIA),and C2-C7 range of motion(C2-C7ROM)of two groups were recorded and compared.The relevant parameters were subjected to the t test,x2 test,univariate and multivariate regression analyses to analyze the risk factors for postoperative CSI.Results The incidence of postoperative CSI was 24.7%,and the results of univariate analysis showed that preoperative C2-C7 Cobb angle,T,-slope,C2-C7SVA and NT were the risk factors that might lead to postoperative CSI(P<0.05).Multivariate logistic regression analysis showed that preoperative C2-C7 Cobb angle,T,-slope,and C2-C7SVA were independent risk factors for sagittal imbalance after posterior C3-6 segmental expansive open-door laminoplasty(P<0.05).Conclusion Patients with MCSM may develop CSI after posterior C3-6 segmental expansive open-door laminoplasty,and preoperative C2-C7 Cobb angle,T,-slope,and C2-C7SVA are independent risk factors for the occurrence of postoperative CSI.
