Long-Term Clinical Effect of Jailed Balloon Technique in True Coronary Artery Bifurcation Lesion

Abstract：Objective To investigate the safety and long-term clinical effects of the jailed balloon technique(JBT)in true coronary artery bifurcation lesion.Methods A total of 210 patients admitted to the Department of Cardiovascular Medicine of Xingyi People's Hospital from April 2016 to April 2018 who had confirmed true coronary artery bifurcation lesion and were randomly divided into JBT group and jailed wire technique(JWT)group.In the JBT group,the true bifurcation artery was pretreated with JBT,that was,the main branch artery(MBA)and side branch artery(SBA)were pretreated with a common balloon,and the branch artery was preembedded with a balloon.JWT group received JWT intervention,that was,ordinary guide wire preembedded SBA lesions.The intraoperative SBA dissection and occlusion,the TIMI blood flow grading of MBA and SBA were observed.The lumen diameter of MBA and SBA and major postoperative adverse cardiovascular events(MACE)were compared between the two groups before percutaneous coronary intervention(PCI),immediately after and during postoperative follow-up.Results The follow-up time of the JBT group and the JWT group was(501.52±48.91)and(495.32±32.72)days,respectively,and there was no statistical difference between the two groups(P＞0.05).At the time of inclusion,the left ventricular ejection fraction(LVEF)of the JBT group was lower than that of the JWT group(P＜0.05).The LVEF was(55.21±7.82)％and(56.72±8.13)％in JWT group and JBT group at the end of follow-up respectively,and showing no statistical difference between the two groups(P＞0.05),suggesting that JBT improved the cardiac function of patients with true bifurcation disease.Angina pectoris and target vessel remodeling occurred in 9 cases(8.57％)and 4 cases(3.83％),19 cases(18.12％)and 12 cases(11.42％)in the JBT group and the JWT group respectively,and there was statistical difference between the two groups(P＜0.05).Compared with preoperative,the immediate minimum lumen diameter of MBA and SBA in JWT group and JBT group was increased(P＜0.05),and the proportion of diameter stenosis was decreased(P＜0.05).Compared with immediately after surgery,the minimum lumen diameter of MBA and SBA in JWT group and JBT group was decreased(P＜0.05),and the minimum lumen diameter was increased(P＜0.05).Compared with the JWT group,the acute gain after SBA in the JBT group was increased(P＜0.05),the internal diameter of the lumen was increased(P＜0.05),the percent diameter stenosis was decreased(P＜0.05),and the late lumen loss was decreased(P＜0.05).Conclusion JBT can further improve the clinical safety and effectiveness of PCI treatment for true bifurcation coronary artery disease,and is an important way to optimize the treatment of true bifurcation coronary artery disease.
