Evaluation Value of Myoglobin Levels in Early Diagnosis and Prognosis of Critically Sepsis Patients

Abstract:Objective To observe the myoglobin(MB)in patients with sepsis and to explore its value in the diagnosis and prognostic prediction of sepsis.Methods A total of 285 suspected infection patients admitted to the First Affiliated Hospital of Zhengzhou University from March 2021 to May 2022 were selected.Baseline data of all patients were collected,and MB within 2 hours of admission were recorded,along with the 28-day survival outcomes.The receiver operating characteristic(ROC)curve was used to assess the area under the curve(AUC)for MB in predicting sepsis mortality,and the Youden index was used to determine the optimal cutoff value for MB.Kaplan-Meier survival curves and log-rank tests were employed to evaluate the cumulative survival rates of patients with different MB.Multivariate logistic regression analysis was conducted to explore the factors associated with poor prognosis in septic patients.Results The non-sepsis group included 39 patients,while the sepsis group included 246 patients.The mortality rate was higher in the sepsis group(P=0.002).The ROC curve assessed the role of MB in the diagnosis of sepsis,with the AUC for 0.768(95%CI:0.686-0.850,P<0.001),and sensitivity and specificity of 0.649 and 0.795.Among the 246 sepsis patients,69 died within 28 days.Kaplan-Meier survival curve analysis and log-rank test showed that patients with MB above 227.5 μg·L-1 had lower 28-day cumulative survival rate compared to those with MB below 227.5 μg·L-1.The multivariate logistic regression model predicted that the MB ≥227.5 μg·L-1 was an independent risk factor for mortality in septic patients(OR=3.173,95%CI:1.494-6.741).Conclusion MB is elevated in septic patients,and its elevation is an independent risk factor for 28-day mortality in these patients.It is also one of the effective biomarkers for early diagnosis and prognosis assessment.
