Use of Carbapenem Antibiotics in Patients with Severe Pneumonia Caused by Multidrug-Resistant Bacteria in a Certain Hospital

Abstract:Objective To analyze the rationality of using carbapenem antibiotics in patients with severe pneumonia caused by multidrug-resistant bacteria(MDRO)infection at General Hospital of Pingmei Shenma Group.Methods A retrospective analysis was conducted on the data and information of 1 285 patients with severe MDRO pneumonia admitted to General Hospital of Pingmei Shenma Group from January to December 2023.The use of carbapenem antibiotics in 1 285 patients was statistically analyzed,including medication type,dosage,amount,etc.Based on defined daily dose(DDD),defined daily dose system(DDDs),daily drug cost(DDC),ranking ratio(B/A)and other data,the rationality of medication was comprehensively analyzed.Results A total of 1 285 patients with severe pneumonia caused by MDRO infection were analyzed,including 624 using imipenem,447 using meropenem,122 using biapenem,and 92 using ertapenem.Among 1 285 patients,317 cases of irrational drug use occurred,among which imipenem and meropenem irrational drug use were mainly due to unreasonable dosage and inappropriate pathogen testing,accounting for 43.45%,32.41%,31.50%,37.80%respectively.Unreasonable use of biapenem and ertapenem was mainly due to unreasonable indications and inappropriate pathogen testing,accounting for 46.67%,30.00%,38.89%,and 44.44%,respectively.There were a total of four carbapenem antibiotics used in 2023,among which imipenem and meropenem were the most commonly used,with a DDDs of 3 233 and a B/A value of 1.00.A total of 97 patients experienced adverse drug reactions,with an incidence rate of about 7.55%.The incidence of digestive system adverse reactions was the highest,followed by liver and kidney function damage.Conclusion The main use of carbapenem antibiotics in General Hospital of Pingmei Shenma Group is imipenem and meropenem,which may result in unreasonable dosage and inappropriate pathogen testing.During medication,special attention should be paid to adverse reactions in the digestive system and liver and kidney function.
