Influencing Factors of Adverse Prognosis in Patients with Psoriasis Vulgaris

Abstract:Objective To explore the adverse prognosis influencing factors and related prediction models in patients with psoriasis(PS)vulgaris.Methods The clinical data of 347 patients with PS vulgaris in Zhumadian Central Hospital from April 2021 to October 2023 were collected and retrospectively studied.The clinical prognosis was counted.The patients were divided into effective group and ineffective group according to the curative effect.The clinical data of the two groups were compared,the influencing factors of prognosis were analyzed,and the logistic prediction model was constructed.Results Among 347 patients with PS vulgaris,117 cases were cured,147 cases were improved,83 cases were invalid,264 cases in the effective group,83 cases in the ineffective group.The levels of C-reactive protein(CRP),neutrophil-lymphocyte ratio(NLR)and platelet-lymphocyte ratio(PLR),the rate of hypoproteinemia,the rate of abnormal liver function,the rate of diabetes mellitus and the complication rate of previous treatment in the ineffective group were higher than those in the effective group(P<0.05).Logistic multivariate analysis showed that elevated PLR,elevated NLR,complex previous treatment,elevated CRP,hypoproteinemia,abnormal liver function,and diabetes mellitus were independent risk factors for poor prognosis in patients with PS vulgaris(P<0.05).The area under the curve(AUC)of predicting poor prognosis in patients with PS vulgaris was 0.858,the sensitivity was 84.06%,and the specificity was 89.13%.Conclusion The increase of PLR,NLR and CRP,the complexity of previous treatment,abnormal liver function,hypoproteinemia and diabetes mellitus are related to the poor prognosis of patients with PS vulgaris.The logistic regression model based on this has good predictive value and can provide guidance for clinical treatment.
