Effect of Different Blood Purification Methods on Complications Related Indicators in Maintenance Hemodialysis Patients

Abstract:Objective To investigate the effects and clinical significance of low-flux hemodialysis(LFHD),high-flux hemodialysis(HFHD),low-flux hemodialysis combined with hemodialysis filtration(LFHD+HDF),and low-flux hemodialysis combined with hemoperfusion(LFHD+HP)on the indexes related to the complications in patients on maintenance hemodialysis.Methods A total of 120 patients who underwent initial blood purification treatment at the Blood Purification Center of Kaifeng People's Hospital from January 2021 to January 2023 were randomly assigned to the LFHD group,HFHD group,LFHD+HDF group,and LFHD+HP group using computer-generated random numbers.The LFHD group received thrice-weekly low-flux hemodialysis treatment.The HFHD group received thrice-weekly high-flux hemodialysis treatment.The LFHD+HDF group received twice-weekly LFHD treatment and once-weekly hemodiafiltration treatment.The LFHD+HP group received twice-weekly LFHD treatment and once-weekly hemoperfusion treatment.Fasting peripheral venous blood was drawn from the patients before the first dialysis and before the first dialysis after 6 months of treatment,and the levels of hemoglobin(Hb),urea nitrogen(BUN),serum creatinine(SCr),parathyroid hormone(PTH),β2-microglobulin(β2-MG),albumin(ALB),triglyceride(TG),total cholesterol(TC),low-density lipoprotein(LDL),C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor α(TNF-α),and other relevant indexes were tested and correlation analyzed.Results After 6 months of treatment,the LFHD group,HFHD group,LFHD+HDF group,and LFHD+HP group showed decreased levels of BUN and SCr in peripheral blood compared to before treatment.The LFHD+HP group showed decreased levels of PTH in peripheral blood compared to before treatment.The HFHD group,LFHD+HDF group,and LFHD+HP group showed decreased levels of β2-MG in peripheral blood compared to before treatment.The LFHD+HP group showed decreased levels of TC,TG,and LDL in peripheral blood compared to before treatment.The LFHD+HDF group and LFHD+HP group showed decreased expression levels of IL-6,CRP,and TNF-α in peripheral blood compared to before treatment.The LFHD+HP group showed increased levels of Hb in peripheral blood,and the LFHD+HDF group and LFHD+HP group showed increased levels of ALB compared to before treatment.The differences were statistically significant(P<0.05).Compared with the LFHD group,HFHD group,and LFHD+HDF group,the LFHD+HP group had the lowest levels of BUN,SCr,PTH,β2-MG,TC,TG,LDL,IL-6,CRP,and TNF-α,and the highest levels of Hb and ALB in peripheral blood after 6 months of treatment.The differences were statistically significant(P<0.05).Correlation analysis showed that the expression levels of IL-6,CRP,and TNF-α in peripheral blood of the LFHD+HP group were negatively correlated with ALB and Hb before and after treatment(P<0.05),and ALB levels were also negatively correlated with BUN levels(P<0.05).Conclusion Compared with LFHD,HFHD,and LFHD+HDF,LFHD+HP can better clear uremic toxins,improve nutrition,anemia,microinflammatory state,and lipid metabolism in hemodialysis patients,and more effectively improve the complications in hemodialysis patients.
