Effect of Spiral Arch Expander Combined with Maxillary Anterior Traction in the Treatment of skeletal Class Ⅲ Malocclusion in Children

Abstract:Objective To explore the clinical effect of combined use of spiral expander and anterior maxillary traction in the treatment of skeletal Class Ⅲ malocclusion in children.Methods A total of 80 children with skeletal Class Ⅲ malocclusion who were treated at Children's Hospital Affiliated to Zhengzhou University from January 2021 to June 2022 were included as study subjects.They were randomly divided into the control group(40 cases,treated with anterior maxillary traction)and the study group(40 cases,treated with spiral expander combined with anterior maxillary traction)using the random envelope method.Evaluate the clinical efficacy of two groups of patients,use cephalometric techniques to detect soft and hard tissue indicators,and refer to the oral health impact scale(OHIP-14)to evaluate the quality of life of patients before and after treatment.Follow up for six months,and calculate the improvement rate,recurrence rate and incidence of adverse reactions of patients'facial features.Results Six months after treatment,the efficacy rate of the study group was higher than that of the control group(P<0.05).After treatment,the values of the upper incisor angle(SNA),the anteroposterior cephalometric angle(ANB),the mandibular plane angle(FH-MP),the maxillary sagittal length(Ptm-A),the menton-to-anterior nasal spine(ANS-Me),and the vertical distance between the maxillary and mandibular skeletal points(Wits value)all increased in both groups.Meanwhile,the lower incisor-mandibular plane angle(L1-MP)and the palatal plane angle(SN-PP)decreased in both groups,with the study group showing more significant improvements than the control group(P<0.05).The study group also had a lower quality of life score compared to the control group(P<0.05).The study group showed a higher rate of facial improvement than the control group(P<0.05),while there was no statistically significant difference in the relapse rate between the two groups(P>0.05).The incidence of adverse reactions did not differ significantly between the two groups(P>0.05).Conclusion The combination of spiral arch expander and maxillary anterior traction has a significant effect on the treatment of skeletal class Ⅲ malocclusion in children,which can quickly improve facial shape,soft and hard tissues,improve quality of life,prevent recurrence,and is safe.
