Effect of Blood Fat Level on the Therapeutic Effect of SGLT-2 Inhibitor in Patients with Early Diabetes Nephropathy

Abstract:Objective To analyze the effect of blood fat levels on the efficacy of sodium-glucose cotransporter 2(SGLT-2)inhibitors in early diabetes nephropathy.Methods A total of 220 patients with early diabetes nephropathy were included in Zhengzhou Yihe Hospital from February 2021 to February 2023.All patients were treated with the SGLT-2 inhibitor daggligin for 12 weeks.Before treatment,blood fat indicators such as triglycerides,total cholesterol,high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)were tested.After treatment,the efficacy was evaluated,and those who showed significant and effective effects were included in the effective group,while those who did not were included in the ineffective group.The blood fat levels of the effective group and the ineffective group were compared.Logistic regression analysis was used to analyze the effect of blood fat levels of early diabetes nephropathy patients on the treatment effect of SGLT-2 inhibitors.Results The total effective rate of 220 patients with early diabetes nephropathy was 81.82%,and the ineffective rate was 18.18%.The levels of urea nitrogen,blood creatinine,triglycerides,total cholesterol,and LDL-C in the ineffective group were higher than those in the effective group,while the glomerular filtration rate and HDL-C levels were lower than those in the effective group,with statistical significant differences(P<0.05).Logistic regression analysis showed that the increased levels of triglycerides,total cholesterol and LDL-C in patients with early diabetes nephropathy were risk factors for ineffective SGLT-2 inhibitor treatment(OR>1,P<0.05),and the increased level of HDL-C was a protective factor(OR<1,P<0.05).Conclusion The blood fat level of early diabetes nephropathy patients will affect the therapeutic effect of SGLT-2 inhibitor.In the future,clinical treatment can be combined with the blood fat level to develop a treatment plan for patients to improve the efficacy.
