Experience of Self-management in Young and Middle-Aged Patients After Tongue Cancer Surgery During Speech Training Based on Self-determination Theory:A Qualitative Study

Abstract:Objective To explore the self-management experience of middle-aged and young patients with tongue cancer treated with speech therapy based on self-determination theory and provide reference for speech therapy intervention measures for middle-aged and young patients with tongue cancer.Methods Qualitative research was conducted and 15 middle-aged and young patients with tongue cancer treated with speech therapy in the Department of Oral and Maxillofacial Surgery of Zhengzhou University First Affiliated Hospital and the Speech Therapy Department were selected as the research subjects.Then in-depth semi-structured interviews were conducted and content analysis was used to analyze,refine and summarizethe interview data.Results Based on the self-determination theory,four themes were identified:insufficient information support,lack of sense of belonging,low participation in self-management plans,reduced competence;unclear self-management goals,lack of autonomy,timely satisfaction of motivational needs helped patients enhance their self-management ability.Conclusion Medical staffs should provide sufficient information support,reasonable plan,clarify management goals,and timely satisfy patients' motivational needs to enhance their sense of belonging,competence,and autonomy so as to improve the self-management ability of middle-aged and young patients with tongue cancer during speech therapy.
