Expression Level of ESM-1 in Alveolar Lavage Fluid in Patients with Severe Pneumonia Complicated with Acute Respiratory Distress Syndrome and Its Clinical Significance

Abstract:Objective To investigate the correlation between endothelial cell-specific molecule 1(ESM-1)expression level in alveolar lavage fluid and the severity and prognosis of severe pneumonia complicated with acute respiratory distress syndrome.Methods A total of 132 patients with severe pneumonia with acute respiratory distress syndrome in the First Affiliated Hospital of Xinxiang Medical University were enrolled and divided into survival group and death group according to the therapeutic efficacy.At admission,the baseline data and the expression level of ESM-1 in alveolar lavage fluid between the two groups were analyzed and compared.The influencing factors of poor prognosis in patients with severe pneumonia with acute respiratory distress syndrome were analyzed by logistic regression.The predictive efficacy was analyzed using the ROC curve.Results On admission,when compared with survival group,the leukocyte,neutrophil,C-reactive protein,procalcitonin,acute physiology and chronic health status score Ⅱ scores and expression level of ESM-1 in the alveolar lavage fluid in death group were significantly increased(P<0.05),while OI index was significantly decreased(P<0.05).The expression levels of ESM-1 in alveolar lavage fluid in mild,moderate and severe of severe pneumonia with acute respiratory distress syndrome was increased gradually,with a statistically significant upward trend(F=18.223,P<0.05).The oxygenation index index,procalcitonin and ESM-1 were independent risk factors for poor prognosis in patients with severe pneumonia with acute respiratory distress syndrome(P<0.05).The areas under the curve of oxygenation index,procalcitonin and ESM-1 predicting poor prognosis of severe pneumonia with acute respiratory distress syndrome were 0.879,0.815 and 0.943,respectively.Conclusion The expression level of ESM-1 in alveolar lavage fluid of patients with severe pneumonia and acute respiratory distress was related to the severity of the disease and poor prognosis.
