Prognostic Value of Serum CXCL12 and CCCK-18 Levels in Patients with Acute Myocardial Infarction Complicated with Diabetes After PCI

Abstract:Objective To investigate the prognostic value of serum CXC chemokine ligand 12(CXCL12)and caspase-cleaved cytokeratin-18(CCCK-18)levels in patients with acute myocardial infarction complicated with diabetes mellitus after percutaneous coronary intervention(PCI).Methods A total of 92 cases with acute myocardial infarction complicated with diabetes who underwent PCI in the 988th Hospital of the People's Liberation Army Joint Logistic Support Force from September 2021 to September 2023 were selected,and 92 patients with acute myocardial infarction complicated with diabetes were divided into poor prognosis group(34 cases)and good prognosis group(58 cases)according to the occurrence of cardiovascular adverse events within 3 months after surgery.The expression levels of CXCL12 and CCCK-18 in serum were detected by ELISA.The predictive value of serum CXCL12 and CCCK-18 levels in the prognosis of acute myocardial infarction patients with diabetes mellitus after PCI was analyzed by receiver operating characteristic(ROC)curve.Binary logistic regression analysis was used to investigate the prognostic factors of PCI in AMI patients with diabetes mellitus.Results The levels of CXCL12 and CCCK-18 in poor prognosis group were significantly higher compared with good prognosis group(P<0.05).The AUC of serum CXCL12 and CCCK-18 levels alone and jointly predicting the prognosis of PCI in AMI patients with diabetes were 0.782,0.859 and 0.926,respectively.Number of lesions(OR=2.588),LVEF(OR=2.380),CXCL12(OR=2.835)and CCCK-18(OR=3.377)were the prognostic factors of PCI in acute myocardial infarction patients with diabetes mellitus(P<0.05).Conclusion Serum CXCL12 and CCCK-18 are highly expressed in acute myocardial infarction cases with diabetes mellitus and poor prognosis after PCI.They are the influencing factors for the prognosis after PCI.The combined detection of CXCL12 and CCCK-18 has a higher predictive value for the prognosis of cases after PCI.
