Predictive Value of Combined Detection of Thromboelastography Parameters and Coagulation Indicators for Bleeding Risk in ICU Critically Ill Patients

Abstract:Objective To explore the predictive value of combined detection of thromboelastography(TEG)and parameters and coagulation indicators for bleeding risk in ICU critically ill patients.Methods A total of 200 ICU critically ill patients induced by infectious lesions admitted to the of Zhumadian Central Hospital from January 2021 to January 2024 were selected and they were divided into bleeding group(98 cases)and non bleeding group(102 cases)according to whether there were bleeding symptoms during the ICU period.The baseline data,TEG parameters[coagulation reaction time(R),blood coagulation time(K),coagulation angle(Angle),maximum amplitude(MA),comprehensive coagulation index(CI)],and coagulation indexes[platelet(PLT)count,prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen(Fib)]were compared between the two groups.Binary logistic regression was used to analyze the independent influencing factors of bleeding in ICU critically ill patients,and receiver operating characteristic(ROC)curves was used to analyze the predictive value of TEG parameters and coagulation indicators combined detection for bleeding risk in ICU critically ill patients.Results The acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)score in the bleeding group was higher than that in the non bleeding group,K and PT were longer than those in the non bleeding group,Angle,MA,CI,PLT,and Fib were lower than those in the non bleeding group(P<0.05).Binary logistic regression analysis showed that,after adjusting for APACHE Ⅱ score,Angle,MA,CI,PLT and Fib were independent influencing factors for bleeding in ICU critically ill patients.ROC curve analysis showed that,the area under the curve(AUC)of Angle,MA,CI,PLT and Fib combined detection for predicting bleeding risk in ICU critically ill patients was 0.946,which was higher than above indicators detection alone 0.863,0.852,0.880,0.854,0.890,respectively(P<0.05).Conclusion TEG parameters and coagulation indicators have certain predictive value for bleeding in ICU critically ill patients.The combined detection of Angle,MA,CI,PLT and Fib has high predictive performance,which can provide reference for the development of targeted intervention plans in clinical practice to reduce the risk of bleeding in ICU critically ill patients.
