Influencing Factors of Abdominal Distension After Non-invasive Positive Pressure Ventilation in Patients with Severe Pneumonia

Abstract：Objective To explore the influencing factors of abdominal distension after non-invasive positive pressure ventilation(NIPPV)in patients with severe pneumonia.Methods A total of 710 patients with severe pneumonia who received NIPPV treatment in Zhoukou Central Hospital from January 2020 to February 2024 were selected as the research subjects.They were divided into the abdominal distension group(215 cases)and the non-abdominal distension group(495 cases)based on whether abdominal distension occurred after NIPPV.Compared with the baseline data,univariate and multivariate logistic regression analyses were used to analyze the related influencing factors of abdominal distension after NIPPV in patients with severe pneumonia.Results There were statistically significant differences between the abdominal distension group and the non-abdominal distension group in terms of age,hypokalemia,mouth breathing,gastrointestinal motility drugs,gastrointestinal decompression,intestinal flora drugs,mechanical ventilation time,and the duration of antibacterial drug use(P＜0.05).Multivariate logistic regression analysis showed that hypokalemia,mouth breathing,prolonged mechanical ventilation time,prolonged use of antibacterial drugs,and non-use of gastrointestinal motility drugs and intestinal flora drugs were independent risk factors for abdominal distension after NIPPV in patients with severe pneumonia(P＜0.05).Conclusion Hypokalemia,mouth breathing,prolonged mechanical ventilation time,prolonged use of antibacterial drugs,and non-use of gastrointestinal motility drugs and intestinal flora drugs may have an impact on secondary abdominal distension after NIPPV in patients with severe pneumonia.Clinical vigilance should be enhanced and corresponding countermeasures should be taken to reduce the incidence of abdominal distension.
