Analysis of Influencing Factors on Prognosis of Patients with Acute Respiratory Distress Syndrome

Abstract:Objective To explore the prognostic factors of patients with acute respiratory distress syndrome(ARDS).Methods A total of 240 ARDS patients admitted to Anyang District Hospital from January 2019 to December 2022 were selected as the study objects.The 28 days prognosis of the patients was analyzed and divided into survival group and death group.The clinical data of survival group and death group were compared.The prognostic factors of ARDS patients were analyzed by logistic regression,and the predictive value of serum histone deacetylase 2(HDAC2)and Toll-like receptor 2(TLR2)levels and acute physiology and chronic health evaluation Ⅱ score(APACHE Ⅱ)in ARDS patients was analyzed by receiver operating characteristic(ROC)curve and area under the curve(AUC).Results Among 240 patients with ARDS,189 patients survived and 51 patients died.Univariate analysis showed that there was no statistical difference in age,gender,body mass index,smoking history,drinking history,hypertension history,diabetes history,number of hyperlipidemia,causative factors,triglyceride(TG)and total cholesterol(TC)values between the two groups(P>0.05).The number of patients with APACHE Ⅱ score ≥20 points and serum HDAC2 and TLR2 levels in death group were higher than those in survival group(P<0.05).Multivariate logistic regression analysis showed that APACHE Ⅱ score(OR=3.854,95%CI:1.608-9.236),serum HDAC2(OR=4.289,95%CI:1.807-10.179),serum TLR2(OR=5.387,95%CI:1.994-14.552)were the prognostic factors of ARDS(P<0.05).ROC results showed that the cut-off values of APACHE Ⅱ score,HDAC2 and TLR2 in predicting the prognosis of ARDS patients were 20 points,155.91 ng·L-1 and 11.43 μg·L-1,respectively.The AUC for predicting the prognosis of ARDS patients were 0.751(95%CI:0.706-0.796),0.853(95%CI:0.806-0.896)and 0.918(95%CI:0.877-0.963),respectively.Conclusion The prognosis of ARDS patients is related to the levels of serum HDAC2,TLR2 and APACHE Ⅱ score,which may be used as reference indicators to predict the prognosis of ARDS patients.
