Application of Sublamina Approach in the Treatment of Lumbar Disc Herniation with Spinal Stenosis by Unilateral Biportal Endoscopic Spinal Canal Decompression

Abstract:Objective To investigate the application of Sublamina approach in the treatment of lumbar disc herniation(LDH)with lumbar spinal stenosis(LSS)by unilateral biportal endoscopy(UBE)spinal canal decompression.Methods A total of 74 patients with LDH combined with LSS who were treated in Shangqiu Hospital of Traditional Chinese Medicine from October 2019 to December 2023 were randomly divided into two groups,37 cases in each group.The control group was treated with microendoscopic discectomy(MED)spinal canal decompression,and the observation group was treated with UBE spinal canal decompression via Sublamina approach.The operation time,bleeding volume and length of hospital stay were compared between the two groups.The numerical rating scale(NRS)scores,Oswestry disability index(ODI),Japanese Orthopaedic Association(JOA)lumbar scores were compared between the two groups before operation and 1 month after operation.The incidence of postoperative complications in the two groups was counted.Results The blood loss in the observation group was less(P<0.05).Compared with before operation,the NRS scores of low back pain and leg pain in the two groups decreased at 1 month after operation,and the NRS scores in the observation group were lower(P<0.05).Compared with before operation,the ODI scores of the two groups decreased and the JOA scores increased at 1 month after operation,and the ODI score of the observation group was lower and the JOA score was higher(P<0.05).The incidence of postoperative complications in the observation group(10.81%)was lower than that in the control group(13.51%),but there was no difference between the two groups(P>0.05).Conclusion Sublamina approach in the treatment of UBE spinal canal decompression in LDH patients with LSS can reduce the amount of intraoperative blood loss,reduce the occurrence of surgical complications,and improve the postoperative back and leg pain and lumbar function.
