Assessment Value of Retinal Vascular Morphology on the Occurrence of Ischemic Stroke

Abstract：Objective To explore the impact of retinal vascular morphology on the occurrence of ischemic stroke.Methods A total of 123 suspected ischemic stroke patients admitted to the First Affiliated Hospital of Xinxiang Medical University from May 2020 to May 2023 were selected as the study subjects,and they were divided into an occurrence group and a non occurrence group based on imaging diagnosis results.All patients collected baseline data at admission and tested parameters related to retinal vascular morphology.The retinal vascular morphology parameters and baseline data were compared between the two groups.Logistic regression was used to analyze the effects of retinal blood vessel morphological characteristics and baseline data on the occurrence of ischemic stroke,and receiver operating characteristics(ROC)curve was drawn to evaluate the value of retinal blood vessel morphological characteristics on the occurrence of ischemic stroke.Results After imaging examination,it was confirmed that among 123 suspected ischemic stroke patients,78 cases were ischemic stroke.They were included in the occurrence group,and 45 cases of undiagnosed ischemic stroke were included in the non occurrence group.The proportion of hypertension,hyperlipidemia,homocysteine,and uric acid levels in the occurrence group were higher than those in the non occurrence group,while the fractal dimension of retinal blood vessels and average retinal artery diameter were lower than those in the non occurrence group,with a statistically significant difference(P＜0.05).The results of logistic regression analysis showed that hypertension,high expression of homocysteine,low expression of retinal vascular fractal dimension,and low expression of mean retinal artery diameter were risk factors for ischemic stroke(P＜0.05).The ROC curve was plotted,and the results showed that the area under the curve(AUC)for evaluating the occurrence of ischemic stroke was 0.738,0.793,and 0.840 for retinal vessel fractal dimension and mean retinal artery diameter alone and in combination,respectively,and the combined evaluation value was higher.Conclusion Low expression of fractal dimension of retinal blood vessels and low expression of mean retinal artery diameter are risk factors for ischemic stroke,and have certain evaluation value for the occurrence of ischemic stroke.
