Expression of ELR,IL-19,Eotaxin-3 in the Nasal Region of Patients with Different Histological Subtypes of Chronic Rhinosinusitis and Nasal Polyps,and Their Correlation with Disease Severity and Efficacy

Abstract:Objective To investigate the correlation between the expression of eosinophil/lymphocyte ratio(ELR),interleukin-19(IL-19),and eotaxin-3 in the nasal region of patients with different histological types of chronic rhinosinusitis with nasal polyps,and the severity of the disease and postoperative efficacy.Methods A total of 200 patients with chronic rhinosinusitis with nasal polyps treated in the First Affiliated Hospital of Xinxiang Medical University from August 2018 to August 2022 were selected and divided into eosinophilic chronic rhinosinusitis with nasal polyps group(69 cases)and non-eosinophilic chronic rhinosinusitis with nasal polyps group(131 cases)according to histological types.The ELR,IL-19 mRNA,eotaxin-3 mRNA,symptom visual analog scale(VAS)score,nasal endoscopy score,and CT score were compared between the two groups of patients.The correlation between ELR,IL-19 mRNA,eotaxin-3 mRNA and symptom VAS score,nasal endoscopy score,and CT score was analyzed.The postoperative efficacy of the two groups of patients was compared,and the ELR,IL-19 mRNA,and eotaxin-3 mRNA of patients with different efficacy were compared.The relationship between ELR,IL-19 mRNA,eotaxin-3 mRNA and efficacy was analyzed.Results The ELR,IL-19 mRNA,eotaxin-3 mRNA,symptom VAS score,nasal endoscopic score,and CT score in the chronic rhinosinusitis with nasal polyps group were higher than those in the non-eosinophilic chronic rhinosinusitis with nasal polyps group(P<0.05).Pearson correlation analysis showed that ELR,IL-19 mRNA,and eotaxin-3 mRNA were positively correlated with symptom VAS scores,nasal endoscopic scores,and CT scores(P<0.001).The improvement rate of olfactory function and the control rate of clinical symptoms in the chronic rhinosinusitis with nasal polyps group were lower than those in the non-eosinophilic chronic rhinosinusitis with nasal polyps group,while the recurrence rate was higher(P<0.05).The levels of ELR,IL-19 mRNA,and eotaxin-3 mRNA in patients with improved olfactory function,controlled clinical symptoms,and recurrence were lower than those in patients with no improvement,uncontrolled clinical symptoms,and no recurrence(P<0.05).Stratified multifactorial logistic regression analysis showed that ELR>0.23,IL-19 mRNA>2.65,and eotaxin-3 mRNA>19.23 increased the risk of unimproved postoperative olfactory function,uncontrolled clinical symptoms,and recurrence.Conclusion The expression of ELR,IL-19,and eotaxin-3 in the nasal region of patients with chronic rhinosinusitis with nasal polyps of different histological types is closely related to the severity of the disease and the postoperative efficacy,and can be used to assist in evaluating the severity of the disease and the postoperative efficacy in patients withchronic rhinosinusitis with nasal polyps.
