Prognostic Value of Serum TgAb,VAP-1,and MK in Predicting the Reoperation of Patients with Recurrent/Persistent Differentiated Thyroid Cancer

Abstract:Objective To investigate the prognostic value of serum thyroglobulin antibody(TgAb),vascular adhesion protein-1(VAP-1),and midkine(MK)in predicting the prognosis of patients with recurrent/persistent differentiated thyroid cancer(DTC)undergoing reoperation.Methods A total of 200 patients with recurrent/persistent DTC in Henan Provincial People's Hospital from January 2017 to January 2021 were selected as the research subjects.All patients underwent reoperation.Patients were grouped according to their prognosis during the 3-year follow-up period after surgery.They were divided into the poor prognosis group(32 cases)and the good prognosis group(168 cases).The clinical data,serum TgAb,VAP-1,and MK levels were compared between the two groups.The impact and predictive value of serum TgAb,VAP-1,and MK on the prognosis of patients with recurrent/persistent DTC undergoing reoperation was analyzed.Results The age,tumor diameter,pathological stage,lymph node dissection of the poor prognosis group were significantly different from those of the good prognosis group(P<0.05).The serum TgAb level and MK level in patients with poor prognosis were higher than those in patients with good prognosis,while the serum VAP-1 level was lower in patients with poor prognosis than in patients with good prognosis(P<0.05).Serum TgAb,VAP-1,and MK remained independent prognostic factors for reoperation in patients with recurrent/persistent DTC(P<0.05).The area under the curve of serum TgAb,VAP-1,and MK combined to predict the prognosis of patients with recurrent/persistent DTC undergoing reoperation was the highest at 0.925,with the best sensitivity of 87.50%,which was higher than that of single and pairwise combinations of these indicators.Conclusion The abnormal expression of serum TgAb,VAP-1,and MK in patients with recurrent/persistent DTC is closely related to the prognosis of patients undergoing reoperation,and TgAb,VAP-1,and MK can be used to assist in evaluating the prognosis of patients undergoing reoperation.
