Risk Factors of Acute Myelosuppression in Breast Cancer Patients After Breast Conserving Surgery Radiotherapy

Abstract:Objective To analyze the risk factors of acute myelosuppression in patients with breast cancer after breast conserving surgery radiotherapy based on multivariate logistic regression analysis,and to put forward target preventive measures.Methods The clinical data of 120 patients with breast cancer who received radiotherapy after breast conserving surgery in the Second Affiliated Hospital of Zhengzhou University from February 2021 to February 2024 were retrospectively analyzed.They were divided into a occurrence group(58 cases)and a non occurrence group(62 cases)according to whether acute myelosuppression occured.The treatment status and general data of the two groups were analyzed by univariate analysis,and the univariate factors of myelosuppression were analyzed by multivariate logistic regression analysis.Results A total of 58 of 120 breast cancer patients with radiotherapy had acute myelosuppression,and the incidence was 48.33%.There were statistically significant differences between the occurrence group and the non occurrence group in age,body mass index,diabetes mellitus,preoperative systemic chemotherapy history,concurrent chemotherapy,tumor invasion of bone marrow,radiation dose,radiation range,treatment times,serum albumin level before treatment,lymphocyte level before treatment and so on(P<0.05).Multivariate logistic regression analysis showed that age ≥60 years old,body mass index<18.5 kg·m-2,diabetes mellitus,preoperative history of systemic chemotherapy,concurrent chemotherapy,tumor invasion of bone marrow,radiation dose ≥50 Gy,radiation range(chest wall+clavicle),treatment times ≥ 25,serum albumin level<40 g·L-1 before treatment,lymphocyte level<2.5× 109 L-1 before treatment were risk factors for acute myelosuppression(P<0.05).Conclusion Breast cancer patients with radiotherapy are easily complicated with acute myelosuppression due to their age,low body mass index,diabetes mellitus,preoperative history of systemic chemotherapy,concurrent chemotherapy,tumor invasion of bone marrow,high radiation dose,wide radiation range,more treatment times,low serum albumin level before treatment,and low lymphocyte level before treatment.Targeted preventive measures should be taken for the above risk factors.
