Difference of Serum CXCL-14 and CCL-18 Expression in Patients with Different Severity of Pulmonary Interstitial Fibrosis and Their Relationship with Pulmonary Function

Abstract:Objective To investigate the difference of serum C-X-C motif chemokine ligand 14(CXCL14)and C-C motif chemokine ligand 18(CCL18)in patients with pulmonary interstitial fibrosis of different severity and their relationship with lung function.Methods A total of 209 patients with pulmonary interstitial fibrosis who came to Zhengzhou Central Hospital from July 2022 to July 2023 were collected as the research group,and were divided into mild group,moderate group and severe group according to severity of pulmonary interstitial fibrosis.A total of 107 healthy people were collected as the control group.Serum CXCL14 and CCL18 levels were detected by ELISA in patients with pulmonary interstitial fibrosis and control group.Pearson method was used to analyze the correlation between serum CXCL14,CCL18 levels,pulmonary interstitial severity and pulmonary function.Multivariate logistic regression was used to analyze the influencing factors of severity of pulmonary interstitial fibrosis.The receiver operating characteristic curve was used to evaluate the diagnostic value of serum CXCL14,CCL18 levels and lung function in different severity of pulmonary interstitial fibrosis.Results The serum levels of CXCL14 and CCL18 in patients in the study group increased(P<0.05).Forced expiratory volume in 1 second(FEV1)/forced vital capacity(FVC)level decreased(P<0.05).Serum CXCL14 level and high resolution CT(HRCT)score were negatively correlated with lung function(r=-0.623,r=-0.513,P<0.05).Serum CCL18 level was negatively correlated with lung function(r=-0.499,P<0.05).There were significant differences in FVC,diffusing capacity of the lungs for carbon monoxide(DLCO),HRCT score,CXCL14,CCL18 and pulmonary function factors among patients with different severity of pulmonary interstitial fibrosis(P<0.05).There was no significant difference in sex,age,body mass index(BMI),basic diseases,smoking history,C-reactive protein(CRP)and other factors among patients with different severity of pulmonary interstitial fibrosis(P>0.05).Compared with the mild+moderate group,the serum levels of CXCL14 and CCL18 in the severe group were higher(P<0.05),while the FEV1/FVC levels were lower(P<0.05).Serum CXCL14,HRCT score,increased levels of DLCO,CCL18 and partial pressure of carbon dioxide in arterial blood(PaCO2),and decreased levels of FEV1/FVC are independent risk factors for patients with severe pulmonary interstitial fibrosis.The area under the curve(AUC)of serum CXCL14,CCL18 and lung function level in the severe group of pulmonary interstitial fibrosis patients were 0.879,0.796,0.795 and 0.937,respectively,with sensitivity of 84.31%,76.47%,74.51%and 94.12%,respectively,and the combined value was higher than that of unilateral diagnosis(Zcombination of the three-CXCL14=2.846,P=0.004;Zeombination of the three-CCL18=3.804,P<0.001;Zcombination of the three-FEV1/FVC=4.618,P<0.001).Conclusion In patients with pulmonary interstitial fibrosis,high levels of serum CXCL14 and CCL18 are expressed,lung function is decreased,and the more severe pulmonary interstitial fibrosis is,the combination of the three has better diagnostic value for pulmonary interstitial fibrosis with different severity.
