Endovascular Treatment Outcomes for Stanford Type B Aortic Dissection Combined with Obstructive Sleep Apnea Syndrome at a Single Center

Abstract:Objective To analyze the clinical manifestations and complication rates after thoracic endovascular aortic repair(TEVAR)in patients with varying degrees of obstructive sleep apnea syndrome(OSAS),and to investigate the impact of OSAS severity on the prognosis of patients with Stanford type B aortic dissection(TBAD)undergoing TEVAR,providing guidance for clinical treatment.Methods A retrospective analysis was conducted on 131 TBAD patients with OSAS who underwent TEVAR at Henan Chest Hospital between January 2019 and December 2023.Patients were divided into mild(29 cases),moderate(81 cases),and severe(21 cases)groups based on portable sleep breathing monitor(PSBM)results.Relevant clinical data from the three groups were collected and analyzed.Results There were no perioperative deaths.Four deaths(3.1%)occurred during follow-up.There were no statistically significant differences among the three groups regarding age,preoperative comorbidities(hypertension,diabetes,stroke,coronary artery disease,combined lower limb ischemia,etc.),and incidence of severe postoperative complications(endoleak,postoperative reintubation,etc.)(P>0.05).However,statistically significant differenceswere observed in waist circumference,body mass index,mortality rate,and distal stent expansion rate(P<0.05).Conclusion For TBAD combined with OSAS,moderate-to-severe OSAS is associated with a significantly increased overall mortality rate and decreased distal stent expansion rate after TEVAR.Active treatment and control of OSAS,along with rigorous postoperative follow-up,are crucial.
