Clinical Characteristics of Lung Cancer with Trousseau Syndrome Manifesting as Acute Cerebral Infarction

Abstract:Objective To analyze the clinical characteristics of lung cancer patients with Trousseau syndrome manifesting as acute cerebral infarction.Methods A retrospective analysis was conducted on the clinical,laboratory,and imaging data of patients with lung cancer complicated by acute cerebral infarction who were hospitalized at the First Affiliated Hospital of Xinxiang Medical University between January 2018 and April 2023.Among these cases,19 were identified as Trousseau syndrome patients.According to the modified Rankin scale(mRS)at discharge,the patients were divided into good prognosis group(mRS ≤3)and poor prognosis group(mRS>3),t test and x2test were used to analyze their data characteristics.Results There were 19 cases(19/157,12.10%)of lung cancer with Trousseau syndrome manifested as acute cerebral infarction,with a median age of 62 years old.There were 15 patients(78.95%)of adenocarcinoma,and 18 cases(94.10%)of stage Ⅳ.The levels of fibrin degradation products(FDP)and D-dimer in most of the patients were higher than those in the normal population.All patients showed multiple acute cerebral infarctions on head MRI.There were 12 cases with a good prognosis and 7 cases with a poor prognosis.The overall mean hemoglobin of the two groups was not statistically significant(P>0.05),there were significant differences in platelet,FDP and D-dimer between the two groups(P<0.05).There was no statistically significant difference in the overall distribution of head MRI infarction involved more than 2 sites between the two groups(P>0.05).Conclusion Lung cancer patients with Trousseau syndrome manifesting as acute cerebral infarction are all advanced stage,and most of the pathological types are adenocarcinoma.The prognosis of patients may be linked to low platelet levels,while high levels of FDP and D-dimer could indicate an increased risk of a worsened prognosis.Additionally,head MRI shows multiple acute cerebral infarction lesions in multiple vascular supply areas and involving more than 2 sites may be related to worse prognosis.
