Characteristics and Influencing Factors of Adverse Blood Transfusion Reactions in Adult Patients in Luoyang,Henan Province

Abstract:Objective To analyze the characteristics and influencing factors of adverse blood transfusion reactions in 100 adult patients in Luoyang,Henan Province,and establish and verify the risk model of adverse blood transfusion reactions in adults.Methods The clinical data of 6 279 adult patients receiving blood transfusion in Luoyang,Henan Province from January 2017 to June 2023 were analyzed by regression.According to the occurrence of adult transfusion adverse reactions,they were divided into occurrence group(100 cases)and non-occurrence group(6 179 cases).The characteristics of adverse blood transfusion reactions in adults were analyzed,the clinical data of the two groups were compared,and the influencing factors of adverse blood transfusion reactions in adults were analyzed by binary logistic regression.The predictive model of adverse blood transfusion reactions in adults was constructed by logistic method.Receiver operating characteristic(ROC)curve was used to evaluate the predictive model.Results Among 100 adult patients with adverse blood transfusion reactions,45 cases(45.00%)of allergic reactions,41 cases(41.00%)of non-hemolytic reactions,and 14 cases(14.00%)of other patients.The occurrence group did not perform platelet antibody and cross-matching test,had blood transfusion history,transfusion times ≥ 2,transfusion types ≥ 2,the proportion of allergic cases was higher than non-occurrence group(P<0.05).Binary logistic regression analysis showed that no platelet antibody and cross-matching detection,allergy,blood transfusion times ≥ 2,and blood transfusion history were the influential factors for the occurrence of adverse blood transfusion reactions in adults(P<0.05).Taking the above influencing factors as predictive variables,a nomogram prediction model was established.The total score of each factor ranged from 78 to 358,and the corresponding risk rate ranged from 0.05 to 0.80.The C-index of the nomogram model was 0.884(95% CI:0.765-0.901),and the correction curve for predicting the risk of adverse blood transfusion reactions in adults was close to the ideal curve(P>0.05).ROC curve results showed that the sensitivity and specificity of the nomogram model for predicting the occurrence of adverse blood transfusion reactions in adults were 89.00%(95%CI:72.13%-95.27%),90.10%(95% CI:73.45%-96.38%),and the area under the curve(AUC)was 0.908(95% CI:0.867-0.951).Conclusion In Luoyang,Henan Province,adult adverse blood transfusion reactions were mainly allergic reactions and non-hemolytic reactions.Adults with more transfusions,allergies,blood transfusion history,and no platelet antibody and cross-matching tests had a higher risk of adverse blood transfusion reactions.The construction of a nomogram model was helpful for early screening of the risk of adverse blood transfusion reactions in adults.
