Influencing Factors for Rheumatoid Arthritis-Related Interstitial Lung Disease

Abstract:Objective To investigate the prevalence of interstitial lung disease(ILD)and risk factors in rheumatoid arthritis(RA)patients.Methods RA patients attending the Department of Rheumatology and Immunology,Huaihe Hospital of Henan University from January 2020 to October 2022 were enrolled,and their demographic characteristics and clinical data were collected,including RA clinical indicators and previous medications,and comorbidities.T-test,x2-test and rank sum test were used to analyze the two groups of data,and multiple logistic regression was used to analyze the risk factors of ILD in RA patients.Results A total of 643 RA patients were included,75.7%(487/643)were female,and the mean age was(58.36±13.54)years,the prevalence of ILD was 29.1%(187/643).Compared with the non-interstitial lung disease(Non-ILD)group,the ILD group had older RA patients,more males,higher disease activity,and higher positive proportion of rheumatoid factors(RF),RF-IgM/G and anti-cyclic citrullinated peptide antibody(ACCP),and had higher prevalence of hypertension,type 2 diabetes and hyperuricemia,and had lower usage of methotrexate(MTX)and hydroxychloroquine(HCQ).Multifactorial logistic regression analysis showed that the older age[OR(95%CI)=1.040(1.022-1.059),P<0.001],positive RF-IgM[OR(95%CI)=2.993(1.298-6.899),P=0.010]were risk factors for ILD in RA patients.Conclusion ILD is a common lung injury in patients with RA and is associated with age and RF-IgM positivity,suggesting that the screening and management of ILD in RA patients should be enhanced.
