Effect of Posterior Urethral Valve Surgery Combined with Circumcision in Preventing Postoperative Urinary Tract Infections

Abstract:Objective To investigate the effect and value of posterior urethral valve resection combined with circumcision in preventing urinary tract infections after posterior urethral valve surgery.Methods A retrospective analysis was conducted on 31 children with posterior urethral valves(PUV)treated at the First Affiliated Hospital of Zhengzhou University between March 2018 and February 2021.Among them,the 19 children treated from March 2018 to February 2020 underwent posterior urethral valve resection alone,while the 12 children treated from March 2020 to February 2021 underwent posterior urethral valve resection combined with circumcision.The postoperative follow-up period was 6 months.During this time,the occurrence of urinary tract infections was monitored via telephone and outpatient follow-up.Six months postoperatively,the children returned to the hospital for imaging examinations including urinary tract ultrasound,voiding cystourethrography(VCUG),and urodynamic studies.Preoperative and postoperative imaging results and urodynamic data were compared between the two groups.Results During the 6-month postoperative period,both the number of children experiencing urinary tract infections and the total number of urinary tract infections episodes were higher in the group that underwent valve resection alone compared to the group that underwent valve resection combined with circumcision.Various bladder function parameters(including maximum detrusor pressure,maximum bladder capacity,bladder compliance,free urinary flow rate,and post-void residual urine volume)showed improvement compared to preoperative values in both groups(P<0.05).However,there was no statistically significant difference in postoperative bladder function between the two groups(P>0.05).Conclusion Performing circumcision concurrently with posterior urethral valve resection can effectively reduce the incidence of postoperative urinary tract infections.However,its effect on improving bladder function or alleviating vesicoureteral reflux appears limited.
