Correlation Between Homocysteine Combined with NT-proBNP and Prognosis in Patients with Heart Failure with Preserved Ejection Fraction

Abstract:Objective To evaluate the correlation between homocysteine(HCY)combined with amino-terminal pro-brain natriuretic peptide(NT-proBNP)and prognosis in patients with heart failure with preserved ejection fraction(HFpEF).Methods A retrospective analysis of 390 patients with HFpEF hospitalized during January 2017 to June 2020 in Endocrinology and Cardiology Department of the First Affiliated Hospital of Henan Polytechnic University.Baseline data of both groups(including gender,age,comorbidities,causes of chronic heart failure,systolic blood pressure,heart rate,body mass index,cardiac function classification),HCY,NT-proBNP,left ventricular ejection fraction(LVEF)of color Doppler ultrasound,long-term medication status.At the same time,the main end point of all-cause death of HFpEF patients was followed up for 1 year.The correlation between HCY and prognosis of HFpEF patients was analyzed.Results With the gradual increase of cardiac function classification,HCY levels gradually increased(P<0.05).HCY was positively correlated with NT-proBNP levels(r=0.494,P<0.05).Patients in the death group had higher HCY levels than those in the non-death group(P<0.05).Logistic regression analysis of factors affecting patient mortality showed that HCY was used as a continuous variable(OR=1.327,95% CI:1.290-2.546,P<0.05)or as a categorical variable(OR=1.643,95% CI:1.477-2.327,P<0.05),all were independent risk factors for death in patients with HFpEF.NT-proBNP was an independent risk factor for death in patients with HFpEF(OR=1.133,95% CI:1.068-1.564,P<0.05).Conclusion There was a significant correlation between HCY,NT-proBNP and HFpEF.Elevated HCY and NT-proBNP is an independent risk factor for death in patients with HFpEF.
