Clinical Efficacy of URL,FURL and PCNL in the Treatment of Ureteral Calculi

Abstract：Objective To exprole the clinical efficacy of ureteroscopic lithotripsy(URL),flexible ureteroscopic lithotripsy(FURL)and percutaneous nephrolithotomy(PCNL)in the treatment of ureteral calculi.Methods The clinical data of 123 patients with ureteral calculi admitted to Zhumadian Central Hospital from January 2022 to January 2023 were retrospectively analyzed.Patients were classified into were divided into URL group(41 cases),F U RL group(42 cases),and PCNL group(40 cases)according to surgical methods,then the hemoglobin(Hb)and oxidative stress indexes,intraoperative and postoperative related indexes,calculi clearance rate and complication occurrence rate were observed in all three groups.Results The Hb,white blood cell count(WBC)and C-reactive protein(CRP)yielded no statistical difference among three groups before operation(P＞0.05).After operation,a decrease in Hb,and an increase in WBC and CRP were detected in all three groups(P＜0.05),and both the increase and decrease were the mildest in FURL group,followed by URL group and PCNL group,with statistical difference(P＜0.05).Compared with URL group and FURL group,PCNL group had lager amount of blood loss,longer duration of surgery,longer length of stay,and longer time setting of urinary catheter and double J tube.Compared with URL group,FURL group had longer duration of surgery(P＜0.05).The calculi clearance rates at postoperative 1,2 and 3 months of FURL group and PCNL group were higher than those of URL group(P＜0.05),while no statistical difference was found between FURL group and PCNL group(P＞0.05).The complication rate was 30.00％ in PCNL group,which was remarkably higher than 9.52％ in FURL group and 12.19％ in URL group(P＜0.05),and the complication rate of FURL group showed no statistical difference with that of URL group(P＞0.05).Conclusion In the treatment of ureteral calculi,URL obtains a poor outcome,and effect of FURL and PCNL is comparable,while the surgical trauma caused by URL and FURL is mild,and the risk of postoperative complications is reduced.
