Anesthetic Effect of Ultrasound-Guided Hip Joint Capsule Peripheral Nerve Block on Patients Undergoing Hip Fracture Surgery

Abstract:Objective To study the effect of ultrasound-guided pericapsular nerve block of the hip joint on the anesthetic effect of hip fracture surgery and postoperative sleep disorder.Methods This study included 64 patients undergoing hip fracture surgery in the Third People's Hospital of Henan Province from December 2020 to December 2022 to carry out a prospective study,which was divided equally into a study group and a control group(32 cases each)by the method of randomized numerical table.The control group received routine tracheal intubation combined with inhalation and general anesthesia,and the study group received ultrasound-guided hip joint capsule peripheral nerve block.The differences in analgesic effect[visual analogue score(VAS)],sleep quality[Pittsburgh sleep quality index(PSQI)],insomnia scale[Athens insomnia scale(AIS)],postoperative rehabilitation quality[quality of rehabilitation scale-15(QoR-15)]and incidence of adverse reactions were compared between the two groups.Results The VAS scores at rest and activity at 30 minutes and 6 hours after block in both groups decreased compared to before block(P<0.05),and the VAS scores at rest and activity at 30 minutes and 6 hours after block in the study group were lower than those in the control group(P<0.05).On postoperative days 1,3,7,and 15,PSQI scores and AIS scores were lower and QoR-15 scores were higher in both groups compared with the preoperative 1 d(P<0.05).PSQI scores and AIS scores were lower than those of the control group,and QoR-15 scores were higher than those of the control group on postoperative days 1,3,and 7 in the study group(P<0.05).The total incidence of adverse reactions and rescue analgesia in the study group were lower than those in the control group(P<0.05).Conclusion Ultrasound-guided periprosthetic nerve block significantly improves postoperative analgesia,facilitates recovery and improves sleep quality in hip fracture patients with a favorable safety profile.
