Application Effect of Evolocumab Combined with Rosuvastatin in Patients with Acute ST-Segment Elevation Myocardial Infarction

Abstract:Objective To explore the application effect of the combination of evolocumab and rosuvastatin in patients with acute ST-segment elevation myocardial infarction(ASTEMI).Methods A total of 150 ASTEMI patients admitted to the Second Affiliated Hospital of Henan University of Science and Technology from October 2020 to October 2022 were selected and divided into 3 groups by a random digital table,with 50 patients in each group.All patients received standard treatment.Group A was given rosuvastatin,group B was given rosuvastatin+evolocumab once every 4 weeks,while group C was given rosuvastatin+evolocumab once every 2 weeks,all of which were treated for 6 months.Blood lipid levels,cardiac ultrasound parameters,6-minute walking test,Kansas city cardiomyopathy survey questionnaire(KCCQ)scores before and after treatment,1-month,3-month,6-month readmission rates and mortality rates were observed and tested by SPSS 25.0 software.Results There were no statistically significant differences in blood lipid levels in group A before and before and after treatment(P>0.05),of which in both group B and group C decreased(P<0.05),and those in group B and group C were lower than group A(P<0.05).After treatment,the left ventricular end-systolic diameter(LVESD)and left ventricular end-diastolic diameter(LVEDD)of the three groups decreased compared to before treatment(P<0.05),of which in both group B and C were lower than group A(P<0.05).After treatment,the left ventricular ejection fraction(LVEF),cardiac output(CO),stroke volume(SV),cardiac index(CI),6-minute walking test distance and KCCQ score in the three groups were increased compared to before treatment(P<0.05),of which in both group B and C were higher than group A(P<0.05).The readmission rates of group B and group C at 6 months were 12.77%and 8.70%,respectively,and the mortality rates were 2.13% and 0%,both lower than those of 29.79%and 12.77%in group A(P<0.05).There were no statistically significant differences in blood lipid levels,cardiac ultrasound parameters,6-minute walking test distance,KCCQ scores after treatment,6-month readmission rates,and mortality rates between group B and group C(P>0.05).Conclusion Evolocumab combined with rosuvastatin can reduce the blood lipid level of ASTEMI patients,improve cardiac ultrasound parameters,improve exercise endurance,enhance cardiac function,and also reduce the 6-month rehospitalization rate and mortality,and the effect of using evolocumab once every 4 weeks and once every 2 weeks is basically the same.
