Correlation Between U-Turn Phenomenon and Heart Failure of Coronary Heart Disease

Abstract:Objective To investigate the correlation between"U-turn"phenomenon and heart failure caused by coronary heart disease.Methods A total of 142 patients with coronary heart disease who were admitted to the Cardiovascular Department of the Affiliated Hospital of Nanjing University of Chinese Medicine from January 2020 to December 2022 and whose 12-lead routine electrocardiogram met the"U-turn"phenomenon were retrospectively selected.Clinical data of the patients were collected and divided into 2 groups according to whether they were accompanied by heart failure.Clinical data and electrocardiogram characteristics between the two groups were compared.The patients with heart failure were divided into 3 groups according to"U-turn"lead,and the correlation between"U-turn"phenomenon and cardiac function grade,eletrocardiogram and cardiac uhrasonography index was analyzed.Results Among 142 patients with coronary heart disease,80 cases(56.3%)were associated with chronic heart failure,and the later the"U-turn",the higher the proportion of heart failure.Spearman correlation analysis showed that"U-turn"lead was positively correlated with QRS duration,left ventricular end-diastolic diameter(LVEDD)and cardiac function grade(P<0.05),and negatively correlated with left ventricular ejection fraction(LVEF)(P<0.05).Conclusion"U-turn"in electrocardiogram can indicate heart failure in patients with coronary heart disease,and the lead of"U-turn"is positively correlated with the severity of coronary heart disease heart failure.
