Efficacy and Safety of Minimally Invasive Tympanoplasty with Otoscope in the Treatment of Chronic Suppurative Otitis Media with Wet Ear

Abstract:Objective To explore the effect of minimally invasive tympanoplasty with otoscope on the treatment and prognosis of patients with chronic suppurative otitis media(CSOM)with wet ear.Methods A prospective study was conducted on 109 patients with CSOM who were admitted to the Second Affiliated Hospital of Henan University of Science and Technology from January 2021 to January 2024.Among them,55 cases were assigned to the study group and 54 cases to the control group according to the random number table method.The patients in the study group underwent minimally invasive tympanoplasty with an ear endoscope,while the patients in the control group underwent microsurgery tympanoplasty.The surgical indicators,rehabilitation conditions,and complications of the two groups of patients were compared.The hearing level,pain level[visual analog scale(VAS)evaluation],quality of life[the Chinese version of the chronic ear surgery(CCES)evaluation]of the two groups of patients before and one month after surgery were compared.The inflammatory factor levels of ear effusion[transforming growth factor-β1(TGF-β1),TGF-β2,tumor necrosis factor α(TNF-α)]of the two groups of patients before and one day,seven days after surgery were compared.The recurrence rate of the two groups of patients six months after surgery was statistically analyzed.Results The surgical time,tympanic membrane healing time,and dry ear time of the study group were shorter than those of the control group,intraoperative blood loss was less than that of the control group(P<0.05).One month after surgery,the air conduction hearing threshold,air bone conduction difference,and high-frequency bone conduction of patients in the study group were lower than those in the control group(P<0.05).On postoperative day 1,the levels of TGF-β1,TGF-β2,and TNF-α in the study group were lower than those in the control group(P<0.05).One month after surgery,the CCES score of the study group patients was higher than that of the control group,and the VAS score was lower than that of the control group(P<0.05).There was no statistically significant difference in the total incidence of complications and 6-month postoperative recurrence rate between the study group and the control group(P>0.05).Conclusion Minimally invasive tympanoplasty using an otoscope can shorten the healing time of the tympanic membrane and dry ear,improve hearing,reduce pain,and enhance the quality of life for patients with wet ear CSOM.
