Clinical Characteristics and Influencing Factors of Patients with Acute ST Segment Elevation Myocardial Infarction Who Failed to Undergo Urokinase Thrombolysis

Abstract:Objective To explore the clinical characteristics and influencing factors of patients with acute ST segment elevation myocardial infarction who failed to undergo urokinase thrombolysis.Methods A retrospective study was conducted on 54 patients with acute ST segment elevation myocardial infarction in Luoyang Petrochemical Hospital from February 2021 to February 2023.Based on the reperfusion status of infarct related vessels(IRA)examined by coronary angiography,they were divided into two groups:thrombolysis failure group(TIMI grade 0-1,8 cases)and thrombolysis reperfusion group(TIMI grade 2-3,46 cases).Two group of general data,laboratory tests,reperfusion treatment related parameters,hospitalization treatment status,major adverse cardiovascular events(MACE)occurrence,and hospitalization time were statistically analyzed,and multivariate logistic regression analysis was used to analyze the influencing factors.Results Among the 54 patients,there were 46 cases of recanalization after thrombolysis,8 cases of thrombolysis failure,and the thrombolysis failure rate was 14.81%.The age of patients in the failed thrombolysis group was lower than that in the thrombolysis reperfusion group(P<0.05),their body mass index was higher than that in the thrombolysis reperfusion group(P<0.05),and their body surface area was larger than that in the thrombolysis reperfusion group(P<0.05).The platelet aggregation rate of patients in the thrombolysis failure group was higher than that in the thrombolysis reperfusion group(P<0.05),activated partial thromboplastin time(APTT)was shorter than that in the thrombolysis reperfusion group(P<0.05),the proportion of thrombus aspiration and stent placement was higher than that in the thrombolysis reperfusion group(P<0.05),and the proportion of blood flow grade Ⅲ after percutaneous coronary intervention(PCI)was lower than that in the thrombolysis reperfusion group(P<0.05).The peak values of creatine kinase(CK)and creatine kinase-MB(CK-MB)were higher than those in the thrombolysis reperfusion group(P<0.05),while left ventricular ejection fraction(LVEF)was lower than that in the thrombolysis reperfusion group(P<0.05).Multivariate logistic regression analysis showed that the influencing factors included age,body surface area,and APTT after thrombolysis(P<0.05).Conclusion Patients with acute ST segment elevation myocardial infarction who failed to undergo urokinase thrombolysis have younger age,larger body surface area,shorter post thrombolytic APTT,larger myocardial infarction area,and poorer cardiac function.The influencing factors include age,body surface area,and post thrombolytic APTT.
