Comparison of the Effects of Unilateral Biportal Endoscopic Discectomy and Percutaneous Endoscopic Transforaminal Discectomy in the Treatment of Lumbar Disc Herniation with Sciatica

Abstract：Objective To compare the clinical efficacy of unilateral biportal endoscopic discectomy(UBED)and percutaneous endoscopic transforaminal discectomy(PETD)in the treatment of lumbar disc herniation(LDH)with sciatica.Methods A total of 74 LDH with sciatica patients admitted to Zhengzhou Central Hospital Affiliated to Zhengzhou University from October 2019 to December 2023 were randomly divided into two groups.The control group was treated with PETD,and the observation group was treated with UBED.The perioperative indicators,visual analogue scale(VAS)score,Oswestry disability index(ODI)and the incidence of surgery-related complications were compared between the two groups.Results The intraoperative blood loss in the observation group was more,and total length of incision was longer,while the intraoperative fluoroscopy frequency was lower(P＜0.05).Compared with those before operation,the VAS scores of low back pain and sciatica and ODI scores of the two groups decreased at 1 and 3 months after operation,and the VAS scores of low back pain and sciatica and ODI scores of the observation group were lower at 1 and 3 months after operation(P＜0.05).The incidence of surgery-related complications was 5.41％in both groups,and there was no difference between the two groups(P＞0.05).Conclusion Compared with PETD,UBED can increase the incision length and intraoperative blood loss,but it does not increase the surgical trauma,and has more advantages in improving the low back pain,sciatica and lumbar function.UBED has high clinical application value in the treatment of LDH with sciatica.
