Risk Factors Analysis of Reperfusion Arrhythmia in Patients with ST-Segment Elevation Acute Myocardial Infarction After PCI

Abstract:Objective To investigate the risk factors of reperfusion arrhythmia(RA)in patients with ST-segment elevation myocardial infarction(STEMI)after percutaneous coronary intervention(PCI).Methods A total of 126 patients with STEMI who received PCI in the Third People's Hospital of Henan Province from January 2019 to February 2023 were selected as the research object,and were divided into RA group(64 cases)and non-RA group(62 cases)according to whether RA occurred after PCI.The clinical data of the two groups were compared,the factors influencing the occurrence of RA after PCI in STEMI patients were analyzed by logistic multivariate regression,and the predictive value of logistic regression model for the occurrence of RA after PCI in STEMI patients was analyzed.Results The time from onset to PCI,arterial pressure waveform,preoperative N-terminal B-type natriuretic peptide precursor(NT-proBNP),cardiac troponin I(cTnI),and lipoprotein-associated phospholipase A2(Lp-PLA2)in the RA group were significantly different from those in the non-RA group(P<0.05).The time from onset to PCI(≤6 hours),abnormal arterial pressure waveform,preoperative NT-proBNP(>137.25 pg·L-1),preoperative cTnI(>47.02 ng·L-1),and preoperative Lp-PLA2(>220.75 μg·L-1)levels were risk factors for RA in STEMI patients after PCI.The area under the curve of the logistic regression model for RA in STEMI patients after PCI was 0.871(95%CI:0.814-0.933),with a predictive sensitivity of 93.45%and a specificity of 72.18%.Conclusion The factors influencing the occurrence of RA after PCI in STEMI patients include the time from onset to PCI,arterial pressure waveform,preoperative NT-proBNP,preoperative cTnI,and postoperative Lp-PLA2.Scientific and effective intervention measures should be developed in clinical work according to relevant influencing factors to reduce the adverse effects of RA.
