Predictive Efficacy of Maximum P-Wave Duration and P-Wave Dispersion for Paroxysmal Atrial Fibrillation in Patients with Essential Hypertension

Abstract:Objective To explore the correlation of maximum P-wave duration(Pmax),P-wave dispersion(Pd)of electrocardiogram with paroxysmal atrial fibrillation(PAF)of essential hypertension(EH)and the predictive efficacy of combined detection of PAF in EH patients.Methods A total of 132 EH patients in Zhengzhou Yihe Hospital from From January 2022 to June 2023 were selected as research objects.The occurrence of PAF within 1 year of follow-up was collected,the ECG detection data were compared between EH with PAF and without PAF patients,and the influence of ECG Pmax and Pd on EH with PAF and the correlation with PAF were analyzed by multi-factor logistic regression and Spearman correlation.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of ECG Pmax and Pd combined detection on EH complicated PAF,and relative risk(RR value)was used to analyze the risk of changes in ECG Pmax and Pd on EH complicated PAF.Results During the 1-year follow-up of 132 EH patients,47 EH patients had PAF and 85 EH patients had no PAF,the incidence of PAF was 35.61%(47/132).The Pmax,Pd and co-corrected QT(QTc)interval in EH with PAF group were(124.67±11.38)ms,(48.91±10.03)ms and(450.93±32.36)ms,respectively,which was higher than those in EH without PAF group(102.48±10.62)ms,(35.67±11.23)ms,(431.12±35.43)ms(P<0.05).Multivariate logistic regression analysis showed that Pmax(OR=3.273,95%CI:1.675-6.394)and Pd(OR=3.796,95%CI:1.814-7.942)were independent influencing factors of PAF in EH patients(P<0.05).Pmax,Pd were positively correlated with EH with PAF(r=0.629,0.683,P<0.05).The area under the curve(AUC)of Pmax and Pd combined detection for predicting EH concurrent PAF was 0.926(95%CI:0.867-0.964),which was higher than that of Pmax and Pd alone detection 0.812(95%CI:0.735-0.875)and 0.834(95%CI:0.760-0.893)(P<0.05);RR values caused by abnormal increase of Pmax and Pd were 5.344(95%CI:2.915-9.800)and 4.531(95%CI:2.866-7.163),respectively(P<0.05).Conclusion Pmax and Pd in electrocardiogram are closely related to PAF in EH.The combined detection of Pmax and Pd has high predictive value for PAF in EH,and can be used as an indicator for clinical prediction of PAF in EH,to guide clinical formulation of treatment and prevention measures to reduce the occurrence of PAF.
