Changes of Serum Levels of SP-D,HMGB1,and TNF-α in TRALI Patients After Acute Massive Hemorrhage Transfusion and Their Predictive Value for Prognosis

Abstract:Objective To explore the serum levels of surfactant protein D(SP-D),high mobility protein B1(HMGB1),tumor necrosis factor-α(TNF-α),and their predictive value for prognosis in patients with transfusion associated lung injury(TRALI)caused by acute massive hemorrhage after transfusion.Methods A total of 100 TRALI patients treated in Zhengzhou People's Hospital from May 2020 to May 2024 were selected as the observation group,while 100 patients with major bleeding who did not experience TRALI were selected as the control group.The differences in serum SP-D,HMGB1,and TNF-αlevels between the two groups were compared.At the same time,the differences in serum SP-D,HMGB1,and TNF-α levels between different clinical characteristics and prognostic patients in the observation group were analyzed,and the value of serum SP-D,HMGB1,and TNF-α levels in predicting patient prognosis was analyzed.Results The serum levels of SP-D,HMGB1,and TNF-α in the observation group were higher than those in the control group(P<0.05).The serum levels of SP-D,HMGB1,and TNF-α in patients with acute physiology and chronic health evaluation score Ⅱ(APACHE Ⅱ)score ≥15 in the observation group were higher than those in patients with APACHE Ⅱ score<15(P<0.05).Serum SP-D,HMGB1,TNF-α were positively correlated with APACHE Ⅱ scores(r=0.349,0.810,and 0.713,P<0.05).The serum levels of SP-D,HMGB1,and TNF-α in the observation group of deceased patients were higher than those in surviving patients(P<0.05).The areas under the receiver operating characteristic(ROC)curve for predicting patient prognosis with serum SP-D,HMGB1,and TNF-α were 0.739,0.895 and 0.600,respectively.Conclusion The serum levels of SP-D,HMGB1 and TNF-α increase in TRALI patients after acute massive hemorrhage transfusion,which is positively correlated with the severity of the patient.Among them,serum SP-D and HMGB1 have certain application value in predicting patient prognosis.
