Comparative Observation of Perioperative Indicators and Prognostic Characteristics in Heart Transplant Recipients with Different Preoperative Pulmonary Artery Pressure Levels

Abstract:Objective To explore the perioperative indicators and prognosis comparison of heart transplant recipients with different preoperative pulmonary artery pressure levels.Methods A total of 120 patients who underwent heart transplantation surgery in Zhengzhou 7th People's Hospital from January 2020 to December 2023 were selected.Based on preoperative mean pulmonary artery pressure(mPAP),the patients were divided into two groups:mPAP≤25 mmHg group and>25 mmHg group.Among them,47 patients were in the mPAP ≤25 mmHg group,and 73 patients were in the mPAP>25 mmHg group.The last preoperative echocardiography,laboratory indicators,perioperative surgical indicators,postoperative complications,and survival status of the two groups were compared.Results The proportion of moderate and above tricuspid regurgitation in the group with mPAP>25 mmHg before surgery was higher than that in the group with mPAP≤25 mmHg(P<0.05).The last preoperative right ventricular(RV)long diameter and N-terminal pro B-type natriuretic peptide(NT-proBNP)in the mPAP>25 mmHg group were higher than those in the mPAP≤25 mmHg group(P<0.05).The incidence of postoperative acute right heart failure was higher in the mPAP>25 mmHg than in the mPAP 25 mmHg group(P<0.05);the incidence of acute rejection,secondary endotracheal intubation,and infection between the mPAP>25 mmHg and mPAP 25 mmHg groups(P>0.05).Conclusion There are differences in preoperative parameters RV,NT proBNP and degree of tricuspid regurgitation,as well as the incidence of acute right heart failure after heart transplantation surgery among patients with different preoperative pulmonary artery pressure levels,but there is no significant difference in postoperative survival.
