Value of Diaphragmatic Function Parameters and Respiratory Mechanics Parameters in Predicting Weaning Outcomes in Patients with Acute Respiratory Distress Syndrome

Abstract:Objective To explore the value of diaphragm function parameters and respiratory mechanics parameters in predicting weaning outcomes in patients with acute respiratory distress syndrome(ARDS)treated with mechanical ventilation.Methods A total of 62 ARDS patients who received mechanical ventilation treatment but failed to withdraw from the machine were selected as the failure group,and 78 patients who received mechanical ventilation treatment and successfully withdrew from the machine for the same disease during the same period were selected as the success group.The differences in diaphragm function parameters and mechanical parameters of the ventilator between the two groups before withdrawal were recorded,and predictive analysis models were established for the indicators with statistical differences to explore the clinical value of these two indicators in predicting the outcome of mechanical ventilation treatment for ARDS.Results There was no statistically difference in platform pressure,driving pressure,and airway resistance between the failed group and the successful group(P>0.05).The peak airway pressure in the failed group was higher than that in the successful group,and the respiratory system compliance in the failed group was lower than that in the successful group,with statistical significance(P<0.05).There was no statistically difference in the variation rate of diaphragm thickness between the failed group and the successful group(P>0.05).The incidence of diaphragm dysfunction in the failure group was higher than that in the success group,and the diaphragm activity in the failure group was lower than that in the success group,with statistical significance(P<0.05).Draw receiver operating characteristic(ROC)curves based on respiratory compliance and peak airway pressure measurements,and predict the area under the ROC curve for the failure of mechanical ventilation treatment in ARDS patients.The ROC curves were 0.855 and 95%CI(0.791-0.920),0.815 and 95%CI(0.743-0.887),respectively.ROC curves based on diaphragm activity and whether there was diaphragm dysfunction was draw.The predicted area under the ROC curve for the failure of mechanical ventilation treatment in ARDS patients was 0.792 and 95%CI(0.718-0.866),0.832 and 95%CI(0.762-0.902),respectively.Conclusion Patients who experience withdrawal failure during ARDS mechanical ventilation treatment have significantly reduced respiratory compliance,increased peak airway pressure,and decreased diaphragm activity and function.These factors can be used as reference indicators for predicting whether patients experience withdrawal failure in clinical practice.
