Evaluation Value of D-Dimer/Platelet Ratio in Prognosis of Elderly Patients with Community-Acquired Pneumonia Complicated with Type Ⅱ Respiratory Failure

Abstract：Objective To explore the value of D-dimer(D-D)/platelet(PLT)ratio(DPR)in the prognosis of elderly patients with community-acquired pneumonia(CAP)complicated with type Ⅱ respiratory failure.Methods A total of 92 elderly patients with CAP complicated with type Ⅱ respiratory failure admitted to tertiary Grade A hospital from August 2021 to February 2023 were selected as the subjects of this study.Blood samples were collected for D-D level and PLT count,and DPR was calculated.According to the 28 d prognosis,the patients were divided into survival group(69 cases)and death group(23 cases).Receiver operating characteristic(ROC)curve was used to evaluate the prognostic value of DPR in elderly patients with CAP complicated with type Ⅱ respiratory failure.Results Among 92 elderly patients with CAP complicated with type Ⅱrespiratory failure,23 died at 28 days,with a mortality rate of 25.00％.The pneumonia severity index(PSI)score and acute physiologic and chronic health evaluation(APACHE Ⅱ)in the death group were higher than those in the survival group(P＜0.05).The PLT in the death group was lower than that in the survival group,and the D-D and DPR in the survival group were higher than that in the survival group(P＜0.05).The area under the curve(AUC)of D-D,PLT and DPR in predicting the prognosis of elderly patients with CAP complicated with respiratory failure were 0.862(95％CI:0.817-0.896),0.721(95％CI:0.671-0.764)and 0.903(95％CI:0.851-0.946),respectively.Conclusion DPR is highly expressed in elderly patients with CAP complicated with type Ⅱ respiratory failure,and it is expected to be used as a reference index to evaluate the prognosis of elderly patients with community-acquired pneumonia complicated with type Ⅱ respiratory failure.
